First Visit


You have chosen a physical therapy center committed to helping you make a permanent change in your life. We take seriously our responsibility to evaluate, problem solve, move and motivate you to optimal health.

You will be greeted by our friendly front desk staff who will ask you to complete some basic paperwork and provide them your insurance information.

As soon as you are done, a physical therapy aide will take you to meet with the physical therapist for your initial evaluation.

The physical therapist will explain your diagnosis and how your treatment program will correct it. Understanding your diagnosis, your surgery or why you have pain is crucial to the success of your rehabilitation program. You should know how your injury occurred, why it has occurred, what needs to be done to fix it and how long it will take. TEAM Physical Therapy will provide you with this education.

In addition, you will be guided through a series of exercises custom designed to treat your condition.

You will leave TEAM with your own photographs to lead you through your home exercise program.

What do I need to bring with me for my first visit?
Doctor’s referral or prescription
Any x-rays, MRI or operative reports you have related to your injury
Insurance card and any claim forms, if required
Comfortable clothes and tennis shoes
